You are invited to participate in this year’s Brighouse Charity Gala on Lane Head Recreation Ground gala field.

Please complete this form in full and submit it as soon as possible to secure your place. Please can you attach a copy of your public liability insurance documents and other documents as soon as possible. The final date for applications will be 31st May 2025.

Please read the following important information and complete each section of the form where applicable. The following answers submitted must be sufficient to satisfy the relevant sections of the above terms and conditions and the documents requested must be uploaded at the end.

As this is a free to enter gala please note there will be a ground fee payable of £20 for charities. Trades will be informed of the amount of their ground fee on application.

Procession

The gala procession will start at Brighouse Central Methodist Church Car Park Brighouse and proceed through the town centre to the gala field via Garden Road Start time 12 noon. All floats assemble at Brow Mills Brighouse by 11.15am Walking groups and walkers must assemble in the Central Methodist Church Car Park.by 11.30 am (Please tick which applies to you)

Gala Sub-Committee Representative

It is a REQUIREMENT of our constitution that each organisation provide a member to serve on one of the gala sub-committees. This will secure any enhancement from a Gala profit to be added to the return of monies raised on the day by your organisation.

Gala Ambassador Nominations

Would you like to have an extra £100 and more promotion for your charity?? If yes then please consider nominating someone from your charity/organisation to be the Gala Ambassador, this person could be male or female over 16 from the Brighouse area who deserves recognition for the amazing work they do. Do they volunteer for you? Are they always willing to lend a hand? Do they go over and above for you? Have they made a difference to a charity, person, family member or the local area etc? Nominations must be returned by the 5th April 2025; these will be reviewed, before the end of April 2025 by Brighouse Ladies Circle and nominees will be contacted directly by Brighouse Ladies Circle. The chosen Gala Ambassador will attend the procession, take part in the opening ceremony, receive a commemorative gift and a cheque for their chosen charity. They may also be invited to attend other events during the year to represent the Gala. (Additional nomination forms can be found on the B.C.G website)

Brighouse Charity Gala Health & Safety Record Sheet – Stalls

Each Charity/Organization will have their own Constitution and appropriate Health & Safety/Risk Assessment. These need to be completed and submitted along with copy of your up to date Public Liability insurance documents and attached to this form.     Please note: Reference to documentation should be a photocopy of insurance policies [no originals]. Any additional documented submission of the organisation risk assessment forms should be signed no longer than 2 weeks before submission.

Site Checks when leaving and how do you record any records of all of the above.
